Whether you are recovering from hip replacement surgery, a stroke, a recent fall or any other health condition requiring rehabilitation, we offer comprehensive programming designed to meet your needs through strong physical, occupational and speech therapy.

Skilled Nursing


Elk River Health and Nursing Center of Ardmore skilled nursing services generally are designed to follow a hospital stay, either on a short or long term basis. Professional nursing care by a dedicated team focuses on the special needs and concerns of each resident. We provide consistent, compassionate nursing care that truly makes our center a nursing home.

Long-term Care/Dementia


Elk River Health and Nursing Center of Ardmore’s commitment to compassionate care is available to individuals diagnosed with dementia and other related cognitive disorders. Care is provided in a safe environment focused on the special needs of these individuals. Residents enjoy appropriate activities and programs supervised by staff that has been specially trained in the care of those who are cognitively impaired. This enhances safety and enjoyment of the residents and provides peace of mind to family members.  

Wound Care 


The Vohra Center of Excellence for Wound Management Program was created to identify and highlight facilities that have developed a superior wound management program. 

 

  Criteria Include:

 

  •  Less than 1% Wound-related Hospitalization Rate 
  •  Designated, accountable Vohra Wound Certified Nurse(s) 7 days per week
  •  Verified Outcomes-focused, Vohra Physician Specialist-led Wound Care Program
  •  Regular Team Meetings with a Focus on Quality and Process Improvement (QAPI)
